Partner with HOM, Inc. to provide safe, stable housing through our Permanent Supportive Housing (PSH) and Rapid Rehousing (RRH) rental assistance programs. HOM works with landlords and property owners to connect eligible households to housing, provide reliable Housing Assistance Payments (HAP), and support successful long-term tenancies.

Permanent Supportive Housing (PSH) Program
HOM’s Permanent Supportive Housing (PSH) program connects eligible households with stable housing while providing landlords with consistent rental assistance payments through Housing Assistance Payments (HAP) contracts.
Landlords participating in the program receive onboarding materials, required forms, and ongoing support to ensure compliance with program requirements and successful tenancy outcomes.
Housing Assistance Payments (HAP) Contract
A Housing Assistance Payments (HAP) Contract is an agreement between HOM, Inc. and a property owner that allows rental assistance to be provided on behalf of an eligible household in the PSH program.
Under a HAP Contract:
- HOM enters into an agreement with the property owner for a specific unit and household
- HOM provides housing assistance payments directly to the owner for the contracted unit
- The tenant resides in the unit while receiving assistance through the PSH program
- Payments are issued according to the terms of the HAP Contract
The HAP Contract applies only to the specific household and unit identified in the agreement.

The Landlord Information Packet provides essential PSH program forms and guidance for participating landlords.
It includes:
- Overview of the Permanent Supportive Housing (PSH) Program
- Request for Tenancy Approval
- Lead Warning Statement
- W-9 for Payee (Taxpayer Identification Request)
- Payment Authorization Form
- Housing Quality Standards (HQS) Landlord Tip Sheet
The PSH program is administered in accordance with applicable federal, state, and local regulations and program requirements.
Rapid Rehousing (RRH) Program
HOM’s Rapid Rehousing (RRH) program helps households quickly secure and maintain stable housing through rental assistance and supportive services.
Participating landlords are provided with onboarding materials, necessary forms, and continued support to help meet program requirements and maintain successful tenancies.
Rental Assistance Agreement (RAA)
A Rental Assistance Agreement (RAA) is an agreement between HOM, Inc. and a property owner that allows rental assistance to be provided on behalf of an eligible household participating in the Rapid Rehousing program.
Under a Rental Assistance Agreement:
- HOM enters into an agreement with the property owner for a specific unit and household
- HOM provides rental assistance payments directly to the owner for the approved unit
- The tenant resides in the unit while receiving assistance through the RRH program
- Payments are issued according to the terms of the Rental Assistance Agreement
The Rental Assistance Agreement applies only to the specific household and unit identified in the agreement.

The RRH Landlord Information Packet provides essential program forms and guidance for participating landlords.
It includes:
- Overview of the Rapid Rehousing (RRH) Program
- Request for Tenancy Approval

The HOM landlord portal provides access to tenancy information, program documents, and support tools for participating landlords.
