  Permanent Supportive Housing Rapid Re-Housing
Targeted to

People with the most severe housing and service needs; including those with disabilities and long or repeated episodes of homelessness who need more support to live stably in their communities.

Individuals and families with moderate housing and service needs to quickly exit homelessness and return to permanent housing.

Barriers to access housing

Participants pay 30% of their adjusted monthly income towards rent and utilities. A pre-determined amount toward rent and utilities they can afford in accordance with applicable program guidelines. The tenant’s share of the rent increases as their income increases until they can afford to entire monthly rent without rental subsidy.
HOM pays
  • The remaining portion of the Contract Rent directly to the landlord.
  • All move-in fees and deposits, excluding pet fees.
  • The remaining portion of the Contract Rent directly to the landlord.
  • All move-in fees and deposits, excluding pet fees.
  • One-time signing bonus up to 1.5 times the monthly rent.
Program duration Participants are assisted for as long as they remain eligible for the assistance. Indefinite. Participants are assisted for as long as is necessary for them to become financially self-sufficient and able to afford the entire Contract Rent with their own income. On average, assistance lasts approximately 6 months.
Provider case management and support services

Depending on need, support services may include:

  • Case management
  • Mental health treatment and counseling
  • Health education and substance use treatment
  • Assistance with daily activities
  • Skills training, socialization, and employment.
  • Transportation
  • Conflict resolution and crisis response

Services are:

  • Client-directed
  • Respectful of individuals’ right to self-determination
  • Voluntary

Appropriate and time-limited services and support focus on:

  • Participants’ housing stability
  • Crisis resolution
  • Providing or assisting connections to resources help improve their safety and well-being
  • Helping to achieve long-term goals.

Services are:

  • Voluntary
Loss Reimbursement

HOM reimburses landlords:

  • Up to two month’s Contract Rent, minus the security deposit, towards unpaid rent, damages or other move-out charges
  • Up to 80% of one month’s rent for vacancy loss

ADOH reimburses landlords:

  • Up to $2,000 (1-bedroom) or up to $3,500 (multi-bedroom), minus security deposit, towards unpaid rent, damages or other move-out charges
  • Up to $1,000 for vacancy loss
