Housing Assistance Payments (HAP) Contract
A Housing Assistance Payments (HAP) Contract is an agreement between HOM, Inc. and a property owner that allows rental assistance to be provided on behalf of an eligible household in the PSH program.
Under a HAP Contract:
- HOM enters into an agreement with the property owner for a specific unit and household
- HOM provides housing assistance payments directly to the owner for the contracted unit
- The tenant resides in the unit while receiving assistance through the PSH program
- Payments are issued according to the terms of the HAP Contract
The HAP Contract applies only to the specific household and unit identified in the agreement.
Landlord Information Packet: Permanent Supportive Housing (PSH)
The PSH Landlord Information Packet provides essential program forms and guidance for participating landlords.
It includes:
- Overview of the Permanent Supportive Housing (PSH) Program
- Request for Tenancy Approval

- W-9 for Payee (Taxpayer Identification Request)
- Payment Authorization Form

The HOM Rapid Re-Housing (RRH) Landlord Packet provides essential program information, forms, and guidance for landlords participating in the RRH program.
It includes:

Move-Out Process
When a tenancy ends, HOM is here to help property owners navigate the move-out process as smoothly as possible. Our team can answer questions about lease completion, final inspections, program requirements, and next steps.
If the tenancy is eligible for damage reimbursement or vacancy loss protections under the applicable housing program, property owners may submit the required documentation for review in accordance with current program guidelines.
